can anyone helpme in constructing the medical statement, that phisician should sign under?
Hereby I confirm that Mr XX (1980-07-08) with driving license nr: xxxXXxx,
does not have contraindications to scuba diving.
is this form correct:
to have contra~
?
or should it be:
to show contra~
?
or sth completely different, I dont have any idea about? ;P

Here in the UK we mainly self certify, see http://www.uksdmc.co.uk/. If it's
commercial/HSE then that's another matter entirely!

Hi,
why not use a preformed Statement?
You'll fnd one at www.gtuem.org That's the website of the German Society
for Diving and Hyperbaric Medicine. Their Statement is primarily german,
but trilingual in the small print.
Apart from the fact that it is already constructed, it looks pretty
impressive :-)
