Could be anxiety, but it could also be a problem with sinus. You don't say
where the headaches is (localised or not). Also if he is anxious on the
first dive, it's possible he ascends qucker on that dive than on subsequent
ones. The fact that the headache eases as he descends on the second dive,
and is mostly gone by the end of the second dive suggests to me that he
might be re-compressing a reverse block and then allowing it time to clear
on the subsequent dive.
Does he have any sinus or ear congestion?

Hi,
Thanks for this. Apparently its a sharp pain rather than dull, always to the
back top of the head (once on the boat). During the dive he tells me its
more apparent behind the eyes - but I guess these are more susceptible to
pressure.
I don't know if he is personally anxious, never seems it to me. We dive a
similar profile and both do the stops. We swap data on our profiles from the
dives to compare so I wouldn't say either of us ascend quickly.
My guess it's the initial anxiety of the first dive in a while and once
settled down he's fine and more in control of his breathing. Getting it on
every first dive seems to point away from the sinus thing, though I could be
wrong as I'm not a doctor.
