medical statement

Hi,
can anyone helpme in constructing the medical statement, that phisician
should sign under?

Hereby I confirm that Mr XX (1980-07-08) with driving license nr: xxxXXxx,

does not have contraindications to scuba diving.

is this form correct:

to have contra~

?

or should it be:

to show contra~

?

or sth completely different, I dont have any idea about? ;P

The person, shop or agency that is asking for or requiring the statement
should be able to tell you the exact language they require -- or they
may has a specific form for you to use.
